Student Registration Notice (No appointment needed)

For those needing to register for the September, 2017 school year and have missed the registration week in the beginning of August, you may do so again during the week of August 28th to September 1st, from 9:00 a.m. - 2:00 p.m.

Please report to Elm Drive Elementary School, at 12 Elm Drive, Millbrook, NY with the following FOUR mandatory items:

1. Original birth certificate with the raised seal OR passport

2. Immunization record

3. School tax bill OR lease agreement OR mortgage statement

4. Utility bill (electric, cable, water or fuel, etc.)

IF YOU DO NOT HAVE THESE FOUR ITEMS, YOU WILL BE ASKED TO RETURN AT A FUTURE DATE.

Please note: In addition to the required documents above, there is a registration packet with forms to be completed. One new form, Registration Affidavit, must be notarized.  If you would like these forms in advance, they are available in the Main Office at all of our buildings, or you may download them from our website: www.millbrookcsd.org.
